Anyone can refer a child for an Early Intervention screening or evaluation. A physician referral is not required.
Child and Family Connections will coordinate an evaluation. If your child is eligible, the family and Early Intervention team will develop an Individualized Family Service Plan identifying goals and recommended services.
Once occupational therapy is included in your child’s IFSP, ask your service coordinator whether Sprout Therapy is available to serve your family.
Children under age three may qualify because of a documented developmental delay, a diagnosed physical or mental condition associated with developmental delay, or certain qualifying risk factors.
Early Intervention screening, evaluation, eligibility determination, service coordination, and IFSP development are provided at no cost to families. Your service coordinator will explain any current insurance or payment requirements that may apply to ongoing services.
